If an Exit Medical is received into Passport 360 from Healthsource, the Passport 360 application will check if the Service Provider employee is linked to a company and a Kumba project.
Once the individual has been verified, the Service Provider Administrator will receive a pop-up message advising them that the individuals need to complete the exit procedures.
Note: The below pop-up message will only appear if NO other medical type is received for an individual from Healthsource within 7 days of the exit medical being sent to Passport 360. If any other medical type is received within 7 days of an exit medical being received into Passport, no pop-up message will appear.
When a Kumba Service Provider logs into the Passport 360 application, the below pop-up message will appear on their screen.
Note: If the individual is linked to multiple projects, it will display in the list for you to view. When completing the exit procedure for one project, the system will automatically unlink the individual from ALL projects. This means that you will not have to upload multiple documents per project.
To complete the termination process, select the view button that appears next to the individuals.
Another screen will pop-up that will direct you to upload the relevant DMR Exit Form for the individual.
Once you have completed the required fields, select the save button.
You will receive a message advising you that the request has been sent to HR for processing.
Just below the document submission, you will have the option to complete the other offboarding steps, namely requesting DMR Exit forms and cancelling the existing Legal Appointments for other employees.
Note: This is not mandatory. The window can be closed after you have uploaded the DMR Exit form for the individual in the termination process.
When you are complete, select save.
The pop-up message will display no individual once all the necessary individuals have followed the correct termination process.
